Why High Protein Diets Cause Kidney Stones (And Who’s at Risk)

Why Some People Can Eat More Meat (and Why Others Make More Stones)

One of the most confusing aspects of kidney stones is that two people can eat the same amount of meat, yet only one develops stones. The difference isn’t willpower, metabolism, or doing something “wrong.” The difference is how their kidneys handle protein.

Some kidneys tolerate a higher protein load without stress, while others show changes in urine chemistry much sooner.

What Happens When We Eat a Lot of Meat Protein

When we eat a lot of meat, poultry, or fish, the body produces acid as part of normal metabolism. This isn’t harmful by itself—it’s a normal process—but that acid has to be handled every single day. How well the body manages that acid makes a big difference in stone risk.

How Acid Affects Calcium (and How the Body Tries to Balance It)

The acid we’re talking about here comes mainly from eating a lot of meat, poultry, and fish. When these foods are broken down, the body produces acid that needs to be managed. The body’s first line of defense is food — specifically fruits and vegetables.

These foods provide natural compounds like citrate and potassium that help the body balance acid and make it easier for the kidneys to keep urine from becoming too acidic.

One Place it Turns is Bone

Calcium is meant to stay in the body. Most of it belongs in bone, where it keeps bones strong and stable. Bone contains calcium salts that can help buffer excess acid, so small amounts of calcium are quietly released from bone into the bloodstream to help maintain balance. This happens slowly and steadily, which is why blood calcium levels can still look normal on lab work.

Once calcium enters the bloodstream, it is filtered by the kidneys as part of normal blood filtration. During this process, calcium temporarily enters the fluid that will become urine. Under normal conditions, the kidneys actively reabsorb most of that calcium back into the bloodstream rather than letting it be lost. Their role is to prevent calcium from being wasted, preserving it so the body can continue to use it — especially to support bone.

The kidneys don’t decide where calcium ultimately goes after that. They act as gatekeepers. By returning calcium to the blood, they keep it available so it can be returned to bone and other tissues that need it.

When acid load stays high, this process changes. Acid stress puts pressure on the kidneys, and in that state, they don’t move calcium back into the bloodstream as efficiently. More calcium slips through and ends up in the urine rather than being retained. That calcium loss means less is available to return to bone over time.

This is one reason urine calcium can rise even when blood calcium looks normal, and calcium intake hasn’t changed. It isn’t that the kidneys are malfunctioning—they’re responding to metabolic stress. Over time, that stress raises kidney stone risk and can quietly contribute to bone loss.

Higher-protein diets have been shown to increase urinary calcium excretion, lower urine pH, and reduce urine citrate, all of which can increase stone risk in susceptible individuals.

How Meat Protein Affects the Kidney

Higher meat protein intake also makes the kidney filters work harder. More protein increases blood flow and pressure inside the kidneys. That extra pressure pushes more calcium into the urine. As a result, more calcium is excreted in the urine rather than being reclaimed by the body.

Why Urine pH Matters

As acid load increases, urine becomes more acidic, resulting in a lower urine pH. More acidic urine increases the risk of stones and makes calcium crystals more likely to form. Acidic urine makes it harder for the kidneys to keep calcium in the body, so more of it gets lost in urine. This effect—higher protein leading to lower pH—has been observed in controlled metabolic studies. Low urine pH can also increase the risk of uric acid stones.

Putting the Pattern Together

For people with “sensitive” kidneys, eating too much protein can make urine more acidic, push calcium out of bones, increase urine calcium excretion, and raise stone risk over time—a pattern repeatedly documented in stone-forming populations and controlled feeding studies.

What do I mean by “sensitive” kidneys?

Several factors can make kidneys less able to handle extra meat without raising urine calcium or stone risk.

Chronic kidney disease

Even early-stage chronic kidney disease reduces the number of functioning filters in the kidneys. That means the remaining filters have to work harder to manage acid and minerals. In this setting, the kidneys are less able to retain calcium when meat intake is high.

Kidney stone history

People who form stones often already handle calcium, acid, or citrate differently at baseline. Their kidneys may produce more acidic urine or lose calcium more easily, making them more sensitive to increases in animal protein.

Naturally low urine pH

Some people naturally make more acidic urine, even without kidney disease. When urine pH is already low, adding more acid from meat makes it harder for the kidneys to hold onto calcium.

Diabetes

Diabetes affects the kidneys long before standard kidney labs show abnormal results. Higher blood sugar increases acid production, lowers urine pH, and impairs the kidneys’ ability to conserve minerals. This makes people with diabetes more likely to lose calcium in urine when meat intake is high.

Obesity

Obesity increases the body’s acid load and promotes low-grade inflammation. Fat tissue isn’t just “stored weight.” It behaves like an organ and releases substances that make the body more acidic. When the body stores excess fat, especially around the belly, the kidneys have to work harder to maintain a balanced acid-base balance. Over time, this can make urine more acidic (lower pH). This puts extra pressure on the kidneys’ ability to conserve calcium, especially with higher intake of animal protein.

Age

As we age, the kidneys become slightly less efficient at regulating acid balance and conserving minerals. This happens gradually and often without obvious lab changes, but it can reduce tolerance for higher meat intake over time.

Does This Mean Eggs and Cheese Too?

When we talk about “too much animal protein” in kidney stone prevention, we are mainly talking about meat, poultry, and fish.

These proteins create the most acid. Eggs count toward total protein, but they create less acid and usually don’t drive urine pH or calcium nearly as strongly in stone formers. Dairy protein behaves differently — it’s neutral to helpful for acid balance and supplies calcium that supports bone health. This is why cutting dairy often backfires in stone prevention.

How Much Protein Is Reasonable

Protein needs are not one-size-fits-all, especially for people who form kidney stones. For most adults, a reasonable daily protein intake falls between 0.36 and 0.91 grams of protein per pound of body weight. Where someone lands in that range depends on how their kidneys respond, not on diet trends or blanket rules.

People who do well at the higher end of this range tend to maintain a stable urine pH and normal urine calcium levels. Others begin to show changes—such as rising urine calcium or more acidic urine—at much lower intakes.

For someone who weighs 175 pounds, this range equals roughly 60 to 160 grams of protein per day. Many stone formers do best closer to the lower or middle part of this range.

Why a 24-Hour Urine Test Is Essential

There is no way to know how someone’s kidneys handle protein, calcium, sodium, or acid without a 24-hour urine collection.

Blood tests do not tell this story. Symptoms do not tell this story. Guessing from diet alone often leads to the wrong advice—including cutting calcium, which is rarely the solution. A 24-hour urine test measures how much calcium is lost, how acidic the urine is, how much sodium is excreted, and how concentrated the urine is overall.

Unfortunately, many stone patients are never offered this test, even though it is one of the most important tools we have to prevent future stones. Please, please, get one ordered from your doctor. I am also an expert in interpreting them to help you have a better office visit with your doctor.

The Takeaway

Protein doesn’t raise urine calcium because someone eats too much calcium. It raises urine calcium by changing how the kidneys manage calcium, especially when combined with high salt and added sugar, and a lower intake of balancing fruits and vegetables. And the only way to see that clearly and personalize prevention is with a 24-hour urine collection.

    Great article Jill ! I’ve never heard any professional explain the effects of high protein intake on the kidneys, let alone explain it in layman’s language that we can all understand.
    I’m still struggling with hypercalciuria despite following the KSD and weighing all my food and tracking calcium, sodium, protein etc. Fluid intake is over 100 oz. a day.
    My urologist referred me to an endocrinologist with whom I’ve had an initial appointment and then I see a nephrologist in late January. No one seems to know what is causing my hypercalciuria (latest 10.65 mmol/d) as my 24 hr. urine values all seem normal except a low PTH. Two different types of thiazides have not worked. Hoping for some clear direction soon. Because no one else really understands my frustration, I just needed to vent to someone who does. Thanks, Jill : -)

      Sometimes your PTH can be normal or low and you still have hyperparathyroidism. Read this article from my mentor but specifically listen to the video I asked him to include at the beginning of the article. He is a much easier person to understand when speaking.         Thank you, Jill. I’ve taken the time to review both the video regarding primary hyperparathyroidism and the original article by Dr. Coe. It seems to indicate that for a diagnosis of primary hyperparathyroidism, your serum calcium readings will come back as high, which mine are not and have not been the past 4 times I’ve had it measured . My results show that I’m right down the middle of the reference range as per LifeLabs. Perhaps I’m an outlier as a person with low PTH, high urine calcium and otherwise normal readings on my hematology and 24 hr. urine collections. I’m doing another urine collection in the new year after 3 in the past year so perhaps something will come out of that. Will have follow up with both endocrinologist and nephrologist in late January. Thanks again for all the education you provide, especially the “Saturday edition” of topics which I always save and reread to keep me on track : -) Daphne

      Hi Sue,
      There is no such thing as oxalate stones, meaning it has to be bound with something and calcium is that thing. So lower oxalate and keep calcium in the urine low. Please read my articles on lower ox.
