## Why Understanding Your Kidneys Changes Behavior

I’ve always believed that people are more willing to change when they understand why they’re being asked to change. Diet advice sticks when it’s tied to what’s actually happening inside the body, not just a list of rules. This approach is what has helped my patients succeed with lifestyle change.

For many people, a kidney stone is a wake-up call. That painful moment creates attention—and attention is where learning happens. It’s like opening the dishwasher and seeing cloudy glasses and grit at the bottom. Nothing broke, but something clearly wasn’t working the way it should.

## What Your Kidneys Are Doing All Day—and Why Stones Form

Before we talk about kidneys, think about a dishwasher you never turn off.

All day long, your kidneys run cycle after cycle in the background. Every sip you take and every bite you eat gets loaded into the dishwasher. Their job is to rinse away waste, decide what your body keeps, and send the rest out—quietly and constantly, without you ever noticing.

Like a dishwasher, kidneys work best when there’s enough water and the load is reasonable. When there isn’t enough water, things don’t rinse clean. When too much “stuff” comes through—salt, oxalate, sugar, excess protein—residue gets left behind. The dishwasher still runs, but it doesn’t clean as well as it used to.

Kidney stones don’t mean the dishwasher is broken. They’re what happens when the system gets overloaded too often, and not enough water is moving through. Grit builds up. Residue hardens. Over time, that buildup turns into stones. This is why increasing fluids is so important for kidney stone formers.

## A Look at What Your Kidneys Are Actually Doing

Your kidneys sit quietly in your lower back, but they are busy all day long. Blood leaves the heart and travels to the kidneys to be filtered (via the renal arteries), carrying water, waste products, and minerals. After the kidney does its work, the blood flows back out cleaner and more balanced and returns to the heart and the rest of the body (via the renal vein).

The outer part of the kidney (the cortex) is where most filtration begins. Deeper inside, urine becomes thicker and more concentrated. This inner area (the medulla) is where stone risk is highest, especially when fluid intake is low or salt intake is high.

Think back to the dishwasher. Less water means residue gets left behind. In the kidney, less water means urine becomes thick. Thick urine allows minerals to stick together instead of being rinsed away. That’s the beginning of stone formation—and why hydration is always goal number one of the Kidney Stone Diet®.

## Where the Kidney Decides What Stays and What Goes

Blood first enters a tiny filter in the kidney (glomerulus), where water and small particles leave the blood and move into the kidney’s tubes. As that fluid travels along (Bowman’s capsule and tubules), the kidney decides what to keep and what to send out as urine.

Think of this as the main rinse cycle in the dishwasher. Everything comes through together, and the kidney decides what gets washed away and what gets saved.

One stretch of this pathway matters more than any other for stone formers (the proximal tubule). This is where the kidney does its “saving.” As fluid moves through the kidney, it pulls useful substances—like water and calcium—back into the bloodstream instead of letting them leave in the urine.

When salt intake is high, that saving process doesn’t work as well. Salt forces more fluid to keep moving forward, and calcium gets swept along with it. Instead of being saved for the blood, calcium is lost in the urine.

That calcium should have stayed in the bloodstream to do important jobs. Instead, it sits in concentrated urine, where it has time to settle, stick to other minerals, and slowly harden. This is one of the main ways calcium stones form.

## Why Low Fluid Makes Everything Worse

Another part of the nephron controls how concentrated urine becomes (loop of Henle). When fluid intake is low, the kidney switches into water-saving mode.

In dishwasher terms, this is a low-water cycle with a full load. The system still runs, but residue continues to build up. In the kidney, darker, thicker urine gives minerals more chances to stick together and form crystals.

The final stretch of the kidney makes last-minute adjustments before urine leaves (the distal tubule and collecting duct). This is where the kidney controls how acidic the urine is and how much calcium and other stone-forming substances remain dissolved rather than stick together.

When urine is too acidic or too concentrated, minerals are more likely to clump and form crystals. By the time urine leaves the kidney, those conditions are already set, so the risk of kidney stones is already determined.

No one makes a stone in a day. But every day, your nephrons respond to the conditions you create.

## How Stones Cause Pain — and Why Prevention Comes First

Once urine is formed, it collects inside the kidney (renal pelvis) and then travels down a narrow tube to the bladder (ureter). If a stone forms and tries to move through that tight space, pain happens.

The pain doesn’t come from the stone forming. It comes from the stone moving—like scraping hardened residue through a narrow pipe.

That’s why stones feel sudden but aren’t. The buildup happens quietly over time. The pain shows up later.

Prevention happens long before that.

## Why the Kidney Stone Diet® Goals Matter to Your Kidneys

When I talk about hydration, sodium, calcium, sugar, protein, and oxalate, I’m not listing random food rules. Each one affects how hard your kidneys have to work and what builds up inside them over time.

### Fluids

When you don’t drink enough, your kidneys try to save every drop of water. Urine gets darker and thicker, like running a dishwasher on a low-water cycle. Minerals don’t rinse away and start sticking together.

### Sodium

When sodium intake is high, the kidneys have a harder time holding on to calcium. Normally, calcium passes through the kidneys during filtration and is then returned to the body for important jobs like keeping your heart beating, your muscles moving, and your nerves working.

Salt and calcium travel together in the kidney. When more salt is excreted in the urine, calcium is dragged along. Even if you’re not eating much calcium, more ends up in the urine where it doesn’t belong.

That extra calcium usually isn’t coming from food. It’s calcium your body already has circulating in the blood and stored in bones.  Over time, that calcium sitting in the urine sticks to other particles and slowly hardens into stones.

This is also why so many stone formers are later found to have bone loss. When calcium keeps leaking into the urine, the body often pulls more from bones to keep blood levels stable.

Lowering sodium protects your kidneys. Cutting calcium does not. In fact, eating too little calcium can make things worse.

Salt doesn’t create calcium—it causes you to lose the calcium your body is trying to protect.

### Calcium

When calcium intake is too low, or calcium is not consumed with meals, oxalate is absorbed rather than blocked in the gut. The kidney then has to deal with it. In thick urine, calcium and oxalate easily find each other. Eating enough calcium lowers stone risk and supports bone health.

### Added Sugar

High sugar intake increases calcium loss in urine and adds extra waste to every wash cycle. The kidney still works, but it has to work harder.

### Meat Protein

High meat intake (anything that runs, swims, or flies is classified as meat) makes urine more acidic, increases calcium loss, and lowers citrate. Acidic urine makes some stones easier to form and harder to dissolve.

### Oxalate

When oxalate intake is high, and there is insufficient fluid or calcium to buffer it, excess crystals can settle in the kidneys. The system becomes overwhelmed. Many of you ate high-oxalate foods (almonds, almond milk, almond flour, and of course, spinach) in generous amounts every single day, along with getting no calcium needs met. And months later, out came a kidney stone.

None of this happens overnight. But the kidney remembers patterns. What you repeat becomes the conditions it has to work in.

## The Takeaway

My job is to teach you how to think—not just how to eat. I want you to understand why your choices matter, what’s happening inside your body, and how what you do now shapes your kidney health over time. When you understand the system, you get to decide what’s worth it and what isn’t.

Taking care of your kidneys isn’t about one perfect day. It’s about being responsible with the choices you repeat—and understanding why those choices matter.
